probelmas con RAID-5

Tengo tres discos ultra scsi 72.8 Gb 10K con software RAID - 5. Red hat 8.
Uno de ellos aparentemente dejo de funcionar. Obtengo el siguiente mensaje:

El test del mismo disco da OK con el cd del fabricante por lo tanto
asumo que es problema de software RAID.
alguna experiencia?
que deberia hacer para agregar el disco al array nuevamente?
